Yoga and Slow Breathing for Blood Pressure

High blood pressure has no symptoms, which is what makes it hard to do anything about. The advice is usually about salt, weight and stress, and none of it comes with something specific to do on a Tuesday evening.

This guide is that specific part. Which breathing rate has a measured effect and why, what 30 trials found when they added yoga to ordinary care, which poses the effective programmes actually contained, and which two things to leave out.

What causes Blood Pressure

The breath is not the calm bit before the practice. It is the part with a mechanism.

Slowing your breathing to around six breaths a minute does something specific and measurable. It increases the sensitivity of the baroreflex, which is the system that reads the pressure in your arteries and adjusts it continuously without you knowing. A less sensitive baroreflex is part of how blood pressure drifts up over years.

In the study that isolated this, slow breathing alone brought systolic pressure down by nearly nine points while baroreflex sensitivity nearly doubled. No poses were involved. That is why the breathing in this program goes first, while you are fresh, rather than last as a wind down.

  • ·Across 30 randomised trials with 2,283 people, yoga lowered systolic blood pressure by 7.95 mmHg and diastolic by 4.93 against control groups.
  • ·Slow breathing alone lowered systolic pressure from 149.7 to 141.1 mmHg while baroreflex sensitivity rose from 5.8 to 10.3 milliseconds per mmHg.

  • ·Alternate nostril breathing was the most common single technique across the effective programmes, and Cobra and Savasana the most common poses.

When to practise and when to get it looked at

Four things decide whether this practice helps or works against you, and all four come out of how the studies behind it were run.

How to practise so it works

  • ·This is an addition to your medical care and any medication, not a replacement. Most of this evidence comes from people who were already on treatment and added yoga on top.
  • ·Never hold your breath, in a pose or between breaths. Holding raises blood pressure, which is the opposite of the point.
  • ·No full inversions. Headstand and shoulderstand raise pressure while you are in them. Legs up the wall is gentle, appears throughout this research and is in every session here.
  • ·Use the breath as your intensity gauge. If it speeds up or you catch yourself holding it, the pose is too much and you come out. That is the whole system.
  • ·Come up slowly from every forward fold, one part of the spine at a time. Standing up quickly is what makes people light headed.

For yoga teachers

Teaching a class for Blood Pressure

The most useful thing you can change in a class for people with high blood pressure is what counts as intensity. Give them the breath as the gauge: if it speeds up or they start holding it, the pose is too much and they come out. That turns every pose into a dose that adjusts itself, and it is far more useful than any instruction about how far to go.

Give the breathing real time and put it at the start. Around six breaths a minute, in for five and out for five, with no pause at either end. That rate has a described mechanism behind it, and the pause is the part to actively cue against because holding the breath pushes pressure the wrong way.

Cue the exit from forward folds as carefully as the entry. Coming up quickly is what makes people light headed, and in this room that matters more than usual. Slowly, one part of the spine at a time, and offer a hand on the thigh.

Two things to leave out and one to keep:

  • ·Leave out full inversions. Headstand and shoulderstand raise blood pressure while you are in them and are generally not recommended with hypertension. Legs up the wall is fine and belongs in every class.
  • ·Leave out rapid forced breathing such as Kapalabhati and Bhastrika. There is no evidence of benefit here and a plausible reason for caution.
  • ·Keep Savasana long. It was the most common pose across the effective programmes, and cutting it to two minutes to fit another sequence in gets the priorities backwards.

The research

What the studies actually found

This is one of the larger evidence bases in the whole field. Thirty randomised trials with 2,283 people pooled together show yoga lowering systolic blood pressure by about 8 mmHg and diastolic by about 5, and a second analysis of 34 trials with 3,125 people finds the same direction and size. Underneath those sit two twelve week trials that used 24 hour monitoring rather than clinic readings, both finding reductions in their yoga groups, and one study that isolated the breathing on its own and measured exactly what it does to the reflex that regulates blood pressure.

Twelve weeks in people with untreated elevated or stage 1 blood pressure, measured with a 24 hour monitor rather than a clinic reading. Twelve weeks with 24 hour monitoring as the main outcome. Diastolic pressure fell 3.9 mmHg across the day and 4.7 overnight in the yoga group, along with mean arterial pressure. The group doing non-aerobic exercise instead showed no significant change.

The trials that measured it ran twelve weeks. Interestingly, longer programmes were not reliably better in the pooled analysis, which suggests that regular practice matters more than accumulating hours.

When should I not start a program like this?

If your resting blood pressure is 180 over 110 or higher, that is the threshold at which exercise programmes should not be started without medical guidance. The same applies to a reading above 140 over 90 that has not been assessed yet, or if you have known heart disease.
