Yoga for the Immune System: What Research Shows

Yoga for the immune system does one thing that shows up consistently in randomised trials: it lowers the pro inflammatory markers that chronic stress keeps elevated. It does not raise your white blood cell count, and it will not stop you catching a cold. The effect that does exist follows total practice time rather than how hard any single session was, which changes what a sensible winter practice looks like.

That distinction matters more than it sounds, because almost everything written about yoga and immunity skips it. The claim is usually that twists massage the organs and inversions drain the lymph, delivered with confidence and no source. The measured picture is narrower, better documented, and in one respect more useful: it tells you how much practice is actually needed.

What yoga does to the immune system, and what it does not

A systematic review of fifteen randomised controlled trials found a consistent pattern of downregulated pro inflammatory markers, most clearly interleukin 1 beta, with supporting signals for interleukin 6 and TNF alpha (Falkenberg 2018). The authors are direct about the limits. The evidence is not entirely consistent, the field is young, and for most immune parameters there is not yet enough to draw firm conclusions.

A larger meta analysis covering 34 trials and 2,219 participants across yoga, tai chi, qi gong and meditation gives the sharper numbers (Morgan 2014). After seven to sixteen weeks there was a moderate reduction in C reactive protein. The reduction in interleukin 6 was small and not statistically significant. The effect on TNF alpha was negligible. And on the measures people usually mean when they say immune boost, CD4 counts and natural killer cell counts, the effect was close to nothing.

So the honest summary is this. Regular practice appears to turn down chronic low grade inflammation. It does not appear to give you more immune cells sitting ready. One finding in that same review points somewhere more interesting: mind body practice seems to improve the immune response to vaccination. That is a response to a challenge, not a resting number, and it fits the pattern better than any lymph drainage story does.

The dose is the finding most people miss

The single most practical result comes from a review looking only at yoga and inflammation across fifteen studies (Djalilova 2019). When the researchers analysed dose, the pattern was clear: a higher total dose, above roughly 1,000 accumulated minutes, produced greater improvement in inflammatory markers.

A thousand minutes is about seventeen hours. At forty minutes a session that is twenty five practices. Three times a week gets you there in around two months. Nothing in that number rewards intensity. It rewards showing up, which is why a practice you can face on a dark evening beats a stronger one you quit in week two.

This is also the best argument against the version of this topic that treats a single session as a treatment. If you practise once because you feel a cold coming on, the research offers you nothing. If you practise through the winter, it offers you something modest and real.

It is worth saying plainly that this number is at odds with how winter actually goes in a yoga room. Rooms are full in September and thin out through January and February, which is precisely the stretch that would carry someone over the 1,000 minute mark. The practice most likely to get you there is not the best one. It is the one with the lowest barrier: short, familiar, no props to find, no decision to make about whether you are up to it tonight. That is why the sequence below ends lying against a wall rather than on something demanding, and why the fifteen minute version is written out at the end of this article instead of being left as an afterthought.

Stress is the mechanism, and it is well mapped

Someone brakes hard in front of you. Your chest tightens, your palms go damp, your jaw sets, and ninety seconds later it is over. That kind of stress actually ramps parts of natural immunity up, which makes sense: it is the response built for a wound that might be about to happen.

An exam week is different. Five days of poor sleep and a knot under the ribs suppresses the cellular side of immunity while antibody production carries on as normal. And a stressor with no end date, a job you cannot leave, a parent you are caring for, suppresses both at once. Those three patterns are not intuition. They come from a meta analysis of more than 300 studies that sorted stressors by how long they last, and found that the duration, not the intensity, decided which part of the immune system gave way (Segerstrom and Miller 2004).

Winter belongs in the third group, and it never announces itself. It is the alarm going off in the dark, the radiator drying your throat out by four in the morning, the sixth Sunday in a row where you did not really go outside, and nothing on the calendar that ends it. There is no moment to point at and nothing that feels like an emergency. That is precisely the profile the meta analysis links to both arms of the immune system being suppressed together.

One detail from that work is worth carrying around, because it cuts against how most people manage this. Subjective reports of stress generally did not predict immune change. The people whose bloodwork looked worst were not reliably the ones who said they felt worst. So the feeling of being fine in November, and the feeling of being run down in February, are both poor guides to when practice would have been doing something.

A study on 50 women, half of them experienced practitioners and half novices, shows what accumulated practice looks like in blood (Kiecolt-Glaser 2010). The novices had interleukin 6 levels 41 percent higher than the experts, and the odds of having detectable C reactive protein were 4.75 times as high. The two groups were matched on age, fitness and abdominal fat, so the difference was not simply that fitter people do yoga.

The part of that study people quote less often is the more instructive half. A single restorative session produced no measurable inflammatory or endocrine advantage over the control conditions at all. Everything the experts had, they had from years, not from the ninety minutes they spent in the lab that day.

Does it actually stop you getting sick

One trial went straight at the question most people are really asking. 413 adults were randomised into eight weeks of mindfulness based stress reduction, eight weeks of matched moderate exercise, or a waitlist control, then tracked through an entire cold season with confirmed viral testing (Barrett 2018).

The meditation group had 112 illness episodes over 1,045 sick days. The exercisers had 120 episodes over 1,010 days. The control group had 134 episodes over 1,210 days. Both active groups also missed less work than the controls. The direction is consistent, and the authors report it carefully: the reduction in incidence for meditation and the reduction in severity for exercise both fell short of the significance threshold they had set in advance, and the trial was probably underpowered.

That is the most honest answer available. Fewer sick days in both active groups, a difference too small for the study to confirm. Anyone telling you a practice prevents colds is going beyond what has been shown.

Why this practice is built the way it is

There is one thing you notice teaching through a winter that no trial has measured. By late January people breathe higher up. Ask a room in October to lengthen the exhale in a twist and most of them can. Ask the same room in February and the exhale stops short, the ribs stay narrow, and the shoulders lift to meet the breath instead of the breath dropping into the body. I have no measurement for that and I would not claim it maps onto any inflammatory marker. It is the reason this practice leans on twists instead of on anything more impressive.

So twists carry most of the forty minutes, four of them. The reason is not that they wring toxins out of anything. It is that twisting takes the ribcage and the diaphragm through their full range, and that range is the first thing lost during a season of sitting indoors and holding yourself against the cold. Breathing well is a plausible route to the outcomes above. Massaging the liver is not.

The practice starts with Cat and Cow on the floor, builds heat through Revolved Chair and the standing sequence, and then comes down for two floor twists. Reclining Belly Twist is the one that costs nothing and does the most, which is why it sits near the end where you will still do it.

Two decisions in this sequence are worth explaining, because both came from running it through our own sequencing checker rather than from how these practices are usually written.

The first is a three breath pause in Thunderbolt Pose, sitting back on the heels, wedged between the Melting Heart backbend and Child's Pose. It looks like filler. It is there because the checker flags a backbend running straight into a forward fold with nothing neutral between them, and three breaths on your heels is the cheapest way to close that gap. In practice this is also the moment where people find out whether their lower back liked the backbend, which is information worth having before the standing block.

The second is bigger. The last shape is Legs Up the Wall and not a headstand, which is what a winter immune sequence traditionally finishes on. The checker rejected the headstand outright, and the reason it gave is a good one: it asks for a jump of five intensity levels while position, category and effort all change in a single step, and it puts the hardest pose of the whole practice after the point where the effort curve is meant to be coming down. Every claim made for inversions applies to five minutes with your legs against a wall. It is also the version you will still be doing in week ten, which given the dose finding is the only version that matters.

What to take from this

It is a Tuesday in early February, dark since half past four, and the mat is rolled up behind the door. Everything above comes down to what happens in the next ten seconds, and none of it argues for a harder practice.

The dose analysis says results track accumulated minutes, not effort. The comparison between experienced and new practitioners puts the difference in years, not in any one session. And the stress meta analysis says the tightness in your shoulders tonight is a poor guide to whether your bloodwork needs you. Three separate findings, all pointing at the same unglamorous thing: a practice small enough that you unroll the mat on the Tuesday you would rather not.

That is why this one ends with your heels against a wall instead of your weight on your head. Not because a headstand is dangerous. Because the version you drop in week three cannot lower anything, and week three is nowhere near where the research puts the effect.

Questions people ask

How often do I need to practise for this to matter

Three times a week is the useful target. The dose analysis points to roughly 1,000 accumulated minutes before inflammatory markers improve measurably, which at forty minutes a session means around twenty five practices. There is no evidence that a single session before a cold does anything.

Do inversions really drain the lymphatic system

The lymphatic system has no pump and does move with muscle contraction and breathing, so movement helps it. But no trial has shown that inverting for a few minutes produces a measurable immune outcome. The documented effects of yoga on immunity run through inflammation and stress, not through drainage.

Should I practise when I already have a cold

Gentle movement is usually fine with symptoms above the neck, and rest is the better choice once you have a fever, body aches or a chest infection. The research here is about prevention over a season, not about treating an illness you already have.

Is yoga better than other exercise for this

Nothing suggests it is. In the trial that compared them directly, moderate exercise and meditation produced similar results, with exercise doing slightly better on severity and meditation slightly better on incidence. Yoga happens to combine both, which is a reasonable argument for it, but not a claim of superiority.

What if I only have fifteen minutes

Cat, Cow, Melting Heart, three breaths on your heels, Child's Pose, Downward Dog, then straight to Reclining Belly Twist for a minute each side, Knees to Chest, and five minutes with your legs up the wall. That is nine shapes, it keeps one twist and one inversion, and it holds the same descending shape as the full practice. Consistency is what the evidence rewards, so a short version you keep is worth more than a long one you abandon.
